
Instructions for the UMS LAN Access Request Form
ALL INFORMATION MUST BE COMPLETE AND LEGIBLE FOR THE REQUEST TO BE PROCESSED
(1) Request Date
Fill in with the current date.
(2) HR Dept #
Check with your payroll/personnel liaison for your department
number. If you do not work for the University, enter the number for the
department that will sponsor you as an affiliate.
(3) Dept. Name
Please give the name for your department that is recorded
in the HR system. Your payroll/personnel liaison can provide this.
(4) Request Type
Are you requesting a new account for a user or modifying
the privileges of an existing account?
(5) Is this person paid from the University’s payroll system?
(Answer only for new ID) If the individual is paid from the University's
payroll system, check 'Yes' and provide the person's employee ID (obtained
from your department’s payroll/personnel liaison). If the person
is not paid from the University's payroll system and is not an active
CU Employee, then check 'No' and fill out the Affiliate Information Sheet.
(6) Name
Please enter the person's full name.
(7) Campus Box Number
Each department has its own box number. Ask the
supervisor or the department payroll/personnel liaison for the correct
number if you are not certain.
(8) Phone Number
Please enter the phone number at work where the person
is most likely to be reached.
(9) Access Information
List access required including drive letter mappings and any special
access. If unsure, who in your department has similar access that you
require? List any special instructions here.
WHAT HAPPENS NEXT?
The user and supervisor sign the certification statements and forward
the request to the UMS LAN Support Group at campus box 50. The Support
Group will create the account and notify the supervisor or sponsor when
it is ready for the user to establish the initial password.
